For each of the questions below, you are to consider the degree to which you have thought and acted upon each item. If the item is something that you have not given any thought to, then your response is toward “1”. If the item is something that you have considered deeply and have made changes in your life to integrate the concept fully, then your response is toward "10”.
 
1. INTUITION - The degree to which you listen to and follow your inner voice.
Are you listening and responding to the quiet voice within you?
I didn’t know I had a voice within.
I found it and I’m beginning to access it.
I allow my inner voice to lead and direct me.
 
2. LEGACY - The degree to which you recognize and utilize your natural gifts and talents.
Are you using your gifts in ways to communicate with the world?

What natural gifts and talents?
I am aware of them and wondering how to use them.
I have integrated my gifts and talents in my life.
 
3. NET WELLNESS - Holistic wellness and balance.
How do your thoughts on aspects of wellness show up in your life?

What does Holistic mean?
I understand my wellness needs and I’m making positive changes.
Holistic health principles are integrated in my life.
 
4. INTEGRITY - Obtaining and following a belief system (values).
What does your value system communicate to those who are watching and listening?
I am not aware of my values.
I am aware of my values and recognize where I need to strengthen them.
My values are fully integrated in my life.
 
5. LIKEABILITY - Your personality.
What does the person you expose to the world say to the world about who you are?
I didn’t know I could modify my personality.
I recognize my personality pros and cons.
I have modified my personality to reflect my best  self.
 
6. NETWORK - The quality and quantity of your relationships.
What do your associations say about you?
I do not have many friends.
I realize that if I enhance my associations I enhance my life.
I have high quality associations that benefit my growth.
 
7. INSPIRATION - Personal internal and external drive.
What drives you – the inner voice of inspiration or the outer voice of motivation?

I need other people to get me motivated.
I am equally motivated and inspired.
I inspire myself and accomplish my goals.
 
8. LIFESTYLE - Your day-to-day activities.
How do your habits speak to who you are?

I have habits?
I am aware of habits that I need to change and I’m ready to address them.
My habits enhance my life.
 
9. NET WEALTH – Abundance.
How do you display your signs of wealth?
I lack abundance.
I understand broad abundance and am working on a plan to achieve.
I have abundance in many areas of my life.
